Heat large pan on medium heat.
1) Saute shallot and garlic in Oil for 2-3 minutes.
2) Add diced green pepper and leek -- saute in oil. Add soy sauce to taste and let cook for 3-4 minutes.
3) Add carrot, green onion and ginger to mixture -- continue to saute for another 2-3 minutes. Add some of the thai chili/garlic sauce to taste, and depending on heat.
4) Add shitake mushrooms and mix -- continue cooking until mushrooms start to get tender (3 or so minutes)
5) Create open space in middle of pan -- add ground chicken and let cook. Flavor with fish sauce & soy sauce. Let cook in middle and then thouroughly mix with all other vegetables. Be sure to make sure chicken is cooked all the way through before serving.
6) Serve family style with head of lettuce and different sauces to taste -- soy, thai chili/garlic, etc.
